DBS Update Service Consent Form

Every revalidation you will be required to complete a Consent to Perform a Status Check form to enable us to check your status online.

If your DBS checks holds new information, British Judo will request you to provide your Original Disclosure certificate from the DBS and follow the Positive Disclosure policy.

You must have signed up to the DBS Update Service within 2-3 weeks of receiving your Enhanced DBS certificate with Child Workforce and Child Barred list to use this service alongside your original certificate (if you do not have your certificate then you will need to request a new DBS check by sending an email to DBS@britishjudo.org.uk).

I confirm that I have an Enhanced DBS check with the Disclosure and Barring Service for a position in a Child or Adult and Child Workforce with Child Barred List. I have previously registered for the DBS Update Service with the certificate number I have provided.

By completing and submitting this form, you are giving your consent for the British Judo Association to perform a status check using the online Update Service with the information you have provided. You must also attach a good quality photo/scan of your enhanced DBS Certificate with Child Workforce and Child Barred List to facilitate this check (If you do not have your certificate you will need to request a new Enhanced DBS Check).

if you do not have a copy of your DBS Certificate and your certificate has previously been registered with us then contact DBS@britishjudo.org.uk to request a DBS Scan bypass code

Max. file size: 64 MB.

Please attach a scan of your Enhanced DBS Certificate with Child Workforce and Child Barred List. Without this we will not be able to do your DBS Update Service check and you will need a new Enhanced Certificate through the BJA (no exceptions)
